BEFORE YOU BEGIN

To complete QDRO Pros, Inc. forms, you will need the following information:

1. Copy of your judgment or Marital Settlement Agreement
   
2. Retirement plan information that includes the plan's exact name, address, and plan administrator name and address
   
3. Current addresses for you and your former spouse
   
4. Social security numbers for you and your former spouse
   
5. Birth dates for you and your former spouse

Attention: QDRO Pros, Inc. will prepare your QDRO in accordance with your MSA or Judgment of Dissolution. Any information you provide that is different from the MSA or Judgment or where the MSA or Judgment are silent, will require written approval by your former spouse.
